Does any 54mm scale fit the Breville Infuser?

No. The nominal size is only the first check. Model-specific locking geometry, basket depth, mounting pattern, or clearance may also matter. Bottomless and replacement portafilters must match the Breville-54 ear family; a 58mm part will not fit.

Is a scale the first upgrade to buy?

Not automatically. The Breville Infuser's stored upgrade order starts with filter basket; fix grinder, basket, dose, or maintenance issues before adding optional tools.
